Step 2: Complete Your Intake Form

This short form is how Jennifer prepares something meaningful specifically for you.

Forms must be submitted at least 3 days before your Circle date. Late submissions may result in a general rather than personalized experience.

A Few Things to Know

Payment confirms your intent to attend
Payments are non-refundable within 48 hours of the event
One rescheduling is permitted with more than 48 hours notice

Jennifer will track all registrations and payments on her end. Once your minimum is confirmed, she will reach out to let you know your circle is locked in.
